Mrs. Horn was born January 15, 1966 in Winder, GA to Johnny Joel Toole and Lois Frances Chambers Toole. She was a retired Licensed Practicing Nurse.

She is survived by her husband: Donald Glen Horn; children: Amber Nichole Horn of Crawford, GA, Courteney Angelina Mander of Crawford, GA, AshLynn Christina Brooke Horn of Crawford, GA, KaLynn Brianna Newbern of Stevens, GA, KatLynn Frances Amos of Crawford, GA, siblings: Mary Lois Shedd of Walton County, GA, Ruby Alice Lapack of Clark County, GA, Tina Teresa Truelove of Oconee County, GA, Mellissa Joyce Toole of Barrow County, GA, Johnny Joel Edward Toole of Oconee County, GA, Johnny Mitchell Toole of Oconee County, GA, Billy Joe Toole of Oconee County, GA and eleven grandchildren.
